- chiropractor – $200 per person / $500 per family
- massage Practitioner and physiotherapist combined – $600 per person
- naturopath – $200 per person / $500 per family
- podiatrist and chiropodist combined – $200 per person / $500 per family
- psychologist – $500 per family
